Surgery of eloquent area gliomas is challenging and requires monitoring of the nearby white fiber tracts. In the present study, we analyzed 102 patients with eloquent region gliomas and discussed the concept of intraoperative dynamic white fiber tract navigation and monitoring. ⋯ The use of intraoperative neuronavigation and neurophysiological assessment can help achieve maximal tumor resection of eloquent area gliomas. Use of the integrated suction stimulator navigator provided dynamic navigation and mapping of the peritumoral eloquent fibers.

Development of an ultrafast brain MR neuronavigation protocol for ventricular shunt placement.
Advancements in MRI technology have provided improved ways to acquire imaging data and to more seamlessly incorporate MRI into modern pediatric surgical practice. One such situation is image-guided navigation for pediatric neurosurgical procedures, including intracranial catheter placement. Image-guided surgery (IGS) requires acquisition of CT or MR images, but the former carries the risk of ionizing radiation and the latter is associated with long scan times and often requires pediatric patients to be sedated. The objective of this project was to circumvent the use of CT and standard-sequence MRI in ventricular neuronavigation by investigating the use of fast MR sequences on the basis of 3 criteria: scan duration comparable to that of CT acquisition, visualization of ventricular morphology, and image registration with surface renderings comparable to standard of care. The aim of this work was to report image development, implementation, and results of registration accuracy testing in healthy subjects. ⋯ The developed and tested sequence of this work allowed accurate intraoperative image registration and provided sufficient parenchymal contrast for visualization of ventricular anatomy. Further investigations will evaluate use of the PICO protocol as a substitute for CT and conventional MRI protocols in ventricular neuronavigation.

Klinger's fiber dissection technique is widely used for studying the anatomy of white matter. Herein, we present a technical description of Klinger's proposed fiber dissection algorithm with neuronavigation assistance which allows for a more accurate determination of the projection of association fibers. ⋯ Electromagnetic navigation is an accurate and useful technique. It allows the researcher the ability to virtually project the association fibers and their cortico-cortical terminations to the surface of the brain, even at the final stages of dissection when the superficial structures are removed. To obtain accurate targeting, it is important to use the appropriate neuronavigation protocol.

Tracking motor and language eloquent white matter pathways with intraoperative fiber tracking versus preoperative tractography adjusted by intraoperative MRI-based elastic fusion.
Preoperative fiber tracking (FT) enables visualization of white matter pathways. However, the intraoperative accuracy of preoperative image registration is reduced due to brain shift. Intraoperative FT is currently considered the standard of anatomical accuracy, while intraoperative imaging can also be used to correct and update preoperative data by intraoperative MRI (ioMRI)-based elastic fusion (IBEF). However, the use of intraoperative tractography is restricted due to the need for additional acquisition of diffusion imaging in addition to scanner limitations, quality factors, and setup time. Since IBEF enables compensation for brain shift and updating of preoperative FT, the aim of this study was to compare intraoperative FT with IBEF of preoperative FT. ⋯ Preoperative tractography after IBEF is comparable to intraoperative tractography and can be a reliable alternative to intraoperative FT.
