Successful extubation conventionally necessitates the passing of spontaneous breathing trials (SBTs) and ventilator weaning parameters. We report successful extubation of patients with neuromuscular disease (NMD) and weakness who could not pass them. ⋯ Continuous volume-cycled NIV via oral interfaces and masks and MAC with oximetry feedback in ambient air can permit safe extubation of unweanable patients with NMD.

Multicenter StudyCombination antibiotic therapy with macrolides improves survival in intubated patients with community-acquired pneumonia.
To assess the effect on survival of macrolides or fluoroquinolones in intubated patients admitted to the intensive care unit (ICU) with severe community-acquired pneumonia (severe CAP). ⋯ Patients with severe community-acquired pneumonia had a low adherence with the 2007 IDSA/ATS guidelines. Combination therapy with macrolides should be preferred in intubated patients with severe CAP.

Randomized Controlled Trial Multicenter Study Comparative Study
Comparison of single-use and reusable metal laryngoscope blades for orotracheal intubation during rapid sequence induction of anesthesia: a multicenter cluster randomized study.
Single-use metal laryngoscope blades are cheaper and carry a lower risk of infection than reusable metal blades. The authors compared single-use and reusable metal blades during rapid sequence induction of anesthesia in a multicenter cluster randomized trial. ⋯ The single-use metal blade was more efficient than a reusable metal blade in rapid sequence induction of anesthesia.

Multicenter Study Clinical TrialAn intervention to decrease complications related to endotracheal intubation in the intensive care unit: a prospective, multiple-center study.
To determined whether the implementation of an intubation management protocol leads to the reduction of intubation-related complications in the intensive care unit (ICU). ⋯ The implementation of an intubation management protocol can reduce immediate severe life-threatening complications associated with intubation of ICU patients.

Multicenter Study Comparative Study Controlled Clinical TrialSurfactant without intubation in preterm infants with respiratory distress: first multi-center data.
Recently in a report of a single center a method has been described to apply surfactant via a thin endotracheal catheter to very low birth weight infants spontaneously breathing with nasal continuous positive airway pressure. We now analyzed available multicenter data. ⋯ A new method of surfactant application was associated with a lower prevalence of mechanical ventilation and better pulmonary outcome. A prospective controlled trial is required to determine whether this approach is superior to standard care.
