Theories of empathy differ regarding the relative contributions of automatic resonance and perspective taking in understanding others' emotions. Patients with the rare syndrome of congenital insensitivity to pain cannot rely on "mirror matching" (i.e., resonance) mechanisms to understand the pain of others. ⋯ In these patients (but not in healthy controls), empathy trait predicted ventromedial prefrontal responses to somatosensory representations of others' pain and posterior cingulate responses to emotional representations of others' pain. These findings underline the major role of midline structures in emotional perspective taking and understanding someone else's feeling despite the lack of any previous personal experience of it--an empathic challenge frequently raised during human social interactions.

Our recent magnetoencephalography study demonstrated that the mu rhythm can reliably indicate sensorimotor resonance during the perception of pain in others (Cheng, Y., Yang, C. Y., Lin, C. P., Lee, P. ⋯ Further, the mu suppression for pain empathy was positively correlated with the scoring on the personal distress subscale of the interpersonal reactivity index only in the female participants. The present findings suggest the existence of a gender difference in pain empathy in relation with the sensorimotor cortex resonance. The mu rhythm can be a potential biomarker of empathic mimicry.

Discussions of empathy in health care offer important ways of enabling communication and interpersonal connection that are therapeutic for the patient and satisfying for the physician. While the best of these discussions offer valuable insights into the patient-physician relationship, many of them lack an action component for alleviating the patient's suffering and emphasize the physician's experience of empathy rather than the patient's experience of illness. ⋯ Clinical empathy can be improved by strategies that address (1) the patient's authority in providing first-person accounts of illness and disability, (2) expanding the concept of empathy to include an action component geared toward relieving patients' suffering, and (3) the potential value of extending empathy to include the social context of illness.

Amygdala hypoactivity to fearful faces in boys with conduct problems and callous-unemotional traits.
Although early-onset conduct problems predict both psychiatric and health problems in adult life, little research has been done to index neural correlates of conduct problems. Emerging research suggests that a subgroup of children with conduct problems and elevated levels of callous-unemotional traits may be genetically vulnerable to manifesting disturbances in neural reactivity to emotional stimuli indexing distress. Using functional MRI, the authors evaluated differences in neural response to emotional stimuli between boys with conduct problems and elevated levels of callous-unemotional traits and comparison boys. ⋯ This finding is in line with data from studies of adults with antisocial behavior and callous-unemotional traits (i.e., psychopaths), as well as from a recent study of adolescents with callous-unemotional traits, and suggests that the neural substrates of emotional impairment associated with callous-unemotional antisocial behavior are already present in childhood.

Absence of embodied empathy during pain observation in Asperger syndrome.
Asperger syndrome (AS) is a neurodevelopmental condition within the autism spectrum conditions (ASC) characterized by specific difficulties in communication, social interaction, and empathy that is essential for sharing and understanding others' feelings and emotions. Although reduced empathy is considered a core feature of ASC, neurophysiological evidence of empathic deficits before and below mentalizing and perspective taking is lacking. We explored whether people with AS differ from neurotypical control participants in their empathic corticospinal response to the observation of others' pain and the modulatory role played by phenomenal experience of observed pain and personality traits. ⋯ Finding no embodiment of others' pain provides neurophysiological evidence for reduced empathic resonance in people with AS and indicates that their empathic difficulties involve not only cognitive dimensions but also sensorimotor resonance with others. We suggest that absence of embodied empathy may be linked to changes at very basic levels of neural processing.
