A COMPARATIVE STUDY OF INTRAMUSCULAR ACETAMINOPHEN VERSUS INTRAMUSCULAR PENTAZOCINE AS LABOUR ANALGESIA IN ZARIA, NORTHWESTERN NIGERIA.
Intramuscular pentazocine is a common labour analgesic in Zaria, Nigeria due to its low cost and availability. Though also cheap and readily available the use of intramuscular acetaminophen is not popular and hence the need for a comparative study. ⋯ Intramuscular acetaminophen gave comparable labour analgesia with fewer side effects when compared with pentazocine.

Case ReportsA patient with acute liver failure and extreme hypoglycaemia with lactic acidosis who was not in a coma: causes and consequences of lactate-protected hypoglycaemia.
Lactate can substitute for glucose as a metabolic substrate. We report a patient with acute liver failure who was awake despite a glucose level of 0.7 mmol/l with very high lactate level of 25 mmol/l. The hypoglycaemia+hyperlactataemia combination may be considered paradoxical since glucose is the main precursor of lactate and lactate is reconverted into glucose by the Cori cycle. ⋯ Severe hypoglycaemia+hyperlactataemia reflects failure of the gluconeogenic pathway of lactate metabolism. The existence of lactate-protected hypoglycaemia implies that patients who present with this metabolic state should not automatically be considered to have sustained irreversible brain damage. Moreover, therapies that aim to achieve hypoglycaemia might be feasible with concomitant hyperlactataemia.

This study aimed to determine if intravenous acetaminophen [paracetamol] (IV APAP) could decrease visual analog pain scores (VAS), opioid exposure and subsequent opioid related adverse effects (nausea, vomiting, constipation) in spinal surgery patients. ⋯ It appears IV APAP can be used effectively as an adjuvant pain management therapy in spinal surgery patients to decrease opioid exposure, but does not necessarily reduce the incidence of opioid related adverse effects or VAS pain scores.
