Göteborg Metoprolol Trial: effects on chest pain.
The effect of metoprolol on chest pain was compared with that of placebo in all randomized patients. The pain score before and 15 minutes after the injection of trial medication was registered and a reduction in chest pain was observed in the metoprolol group. Increasing chest pain after blind injection was observed in only 16 and 9 patients from the placebo and metoprolol groups, respectively. ⋯ Either metoprolol does not induce coronary vasospasm or spasm does not play a role in these patients with definite and suspected acute myocardial infarction as well as unstable angina pectoris. Metoprolol reduced the need for analgesics during the first 4 days and shortened the duration of pain. The effects were similar in patients with early and late treatment, but may depend on initial heart rate, blood pressure and site of infarction.

Penile block for pain relief after circumcision in children. A randomized, prospective trial.
The value of adjunctive penile block for analgesia, after circumcision in children using 0.5 percent bupivacaine has been evaluated in this randomized, prospective trial. Significantly more children without nerve block were found to require analgesics, administered by injection orally, in the first 12 hour period when compared with children given the nerve block. Penile block is a procedure that is safe, simple to achieve, free from complications, and covers the postoperative period when analgesia is much needed.

The analgesic effect of a low dose of alfentanil.
The effect of a single small dose of alfentanil (6 micrograms/kg) on postoperative pain was compared with saline using a double blind study. Pain was assessed using a linear analogue scale and shown to decrease at 2, 5 and 10 minutes after injection of alfentanil (p less than 0.01). ⋯ There were no changes in pain or PE'CO2 in the control group throughout the study. Intravenous alfentanil given to patients in pain provides quick effective analgesia for a short period of time, but respiratory depression may occur.

Randomized Controlled Trial Comparative Study Clinical TrialA controlled trial of pain on skin infiltration with local anaesthetics.
A double-blind, randomised controlled trial was performed to compare the pain experienced on intradermal and subcutaneous injection of lignocaine 1% with adrenaline 1:100,000, lignocaine 1%, procaine 1% and normal saline. A linear analogue pain scale was used to assess pain. ⋯ These differences were significant at p less than 0.05 or greater for all comparisons except procaine 1% and normal saline. In conclusion, procaine 1% appears to be the local anaesthetic of choice from among those tested for intradermal and subcutaneous injection.

Imipramine treatment of painful diabetic neuropathy.
Twelve patients with severe, painful diabetic neuropathy in the lower extremities were treated with imipramine and placebo in a fixed-dose, double-blind, crossover study of five plus five weeks. Seven patients experienced notable improvement while receiving imipramine and none while receiving placebo. The rating of specific symptoms at the end of each treatment period showed a beneficial effect of imipramine on pain, paresthesia, dysesthesia, numbness, and nocturnal aggravation. The plasma levels of imipramine and its metabolite desipramine were significantly higher in patients who benefited from imipramine treatment.
