Case ReportsPainless Posterior Scleritis: A Rare Presentation.
Posterior scleritis is an underdiagnosed condition due to its rarity and is frequently misdiagnosed due to its varied presentation.1 Therefore, it is imperative to know that it can present clinically as exudative retinal detachment, proptosis, disc oedema, choroidal folds, subretinal mass, and myositis. We report a case of 48-year female who presented with gradual, progressive loss of vision in left eye. There were multiple exudative retinal detachments with choroidal folds and disc oedema. ⋯ With oral steroids, she improved clinically. Posterior scleritis is a painful condition, but the patient did not report any pain in or around the globe, which makes this case a unique experience. Could C-Reactive Protein/Albumin Ratio be an Indicator of Activation in Axial Spondyloarthritis?
To evaluate the relationship of C-reactive protein (CRP)/albumin ratio (CAR), neutrophil/lymphocyte ratio (NLR), platelet/lymphocyte ratio with Bath Ankylosing Spondylitis Disease Activity Index (BASDAI) in axial spondyloarthritis (Ax-SpA). ⋯ There is a significant correlation between CAR value and composite score such as BASDAI, which is commonly used to assess systemic inflammation in Ax-SpA. These methods are reliable, recognised by reimbursement institutions and include measurements that physicians can use practically in routine outpatient clinics. Case ReportsManagement of a Firmly Lodged Endotracheal Tube: A Case of High Quality Communication.
This article describes a case of endotracheal tube firmly lodged in the trachea of a patient due to denuded subglottic cartilage and subglottic swelling. A 30-year female, who had a number of previous intubation episodes, presented with reduced consciousness and was intubated in order to protect and maintain her airway. ⋯ Furthermore, this case highlights the importance of a high quality communication using algorithmic surgical management plan to achieve a safe and controlled environment within theatre.
