Surgery endangers the integrity of the body through a continuous stream of noxious stimuli. General anesthesia helps patients cope with the surgery situation. In the first part of our literature review, we present our new knowledge about nociception as described by Sherrington. ⋯ Maintaining the unconscious state created by anesthetics during surgery is only possible by continuously counteracting nociception. Finally, we present the role of the opioid receptor system in antinociception. Understanding all these processes can help expand our knowledge about nociception, pain and formation of consciousness.

Psychogenic erectile dysfunction (pED) is often accompanied by abnormal brain activities. This study aimed to develop an automaticclassifier to distinguish pED from healthy controls (HCs) by identified brain-basedcharacteristics. Resting-state functional magnetic resonance imaging data were acquired from 45 pED patients and 43 HCs. ⋯ This study identified altered regional activity and FC in specific brain regions of pED patients, which might be related to the development of pED. The application of machine learning confirmed the distinctive characteristics of these functional changes in the brain. The high accuracy of our diagnostic model suggested a promising direction for developing objective diagnostic tools for psychological disorders.

Due to the increasing prevalence of depressive and anxiety disorders in youth, a growing interest in the endocannabinoid system (ECS) as a potential alternative target point for treatment arised. This study aimed to investigate whether chronic administration of escitalopram reverses behavioral changes induced by maternal separation in male adolescent Wistar rats and explore the corresponding neurochemical changes in the ECS. The pups were separated from their dams for 360 min daily from postnatal day (PND) 2 until PND 15. ⋯ Escitalopram reversed anxiety-like behavior and attenuated signs of despair behavior. The escitalopram administration has been followed by a decrease in the studied genes expression in the amygdala, the hypothalamus, and the hippocampus, what might suggest that the endocannabinoid system is involved in the mechanism of its action in adolescents. However Western blot analysis did not indicate significant alterations in the protein levels, so more detailed studies are needed to verify this hypothesis.

Towards optimization in the use of hemostatic agents and blood products in the early treatment of patients with traumatic brain injury (TBI).
The treatment of patients with traumatic brain injury (TBI) with subsequently evolving hemostatic failure and hemorrhagic lesion progression remains challenging. New studies highlight windows of opportunity for treatment optimization. ⋯ Mortality in TBI with subsequent evolving hemostatic failure can be reduced through treatment optimization delivering early prehospital high-dose tranexamic acid and in-hospital goal-directed treatment algorithms to timely correct coagulopathy and restore hemostasis.

This review aims to summarize recent developments for the management of severe traumatic brain injury (TBI) in the ICU. Recent advancements in TBI ICU management emphasize a progression toward more multimodal approaches and mitigating secondary brain injury by increased focus on careful systemic management. ⋯ ICP monitoring remains a key component of severe TBI management in ICU. Emerging evidence is slowly changing and improving intensive care and patient outcomes and include both brain-targeted therapies and careful systemic intensive care management.
