Multicenter StudyBlood Pressure Management Goals in Critically Ill Aneurysmal Subarachnoid Hemorrhage Patients in Australia and New Zealand.
Blood pressure (BP) management is common in patients with aneurysmal subarachnoid hemorrhage (SAH) admitted to an intensive care unit. However, the practice patterns of BP management (timing, dose, and duration) have not been studied locally. ⋯ BP management goals are commonly 'prescribed' to aSAH patients admitted to an intensive care unit in Australia and New Zealand, but BP management goal setting was not associated with improved outcomes in the adjusted analysis.

Machine learning and deep learning techniques offer a promising multidisciplinary solution for subarachnoid hemorrhage (SAH) detection. The novel transfer learning approach mitigates the time constraints associated with the traditional techniques and demonstrates a superior performance. This study aims to evaluate the effectiveness of convolutional neural networks (CNNs) and CNN-based transfer learning models in differentiating between aneurysmal SAH and nonaneurysmal SAH. ⋯ CNN-based transfer learning models can accurately diagnose the etiology of SAH from computed tomography images and is a valuable tool for clinicians. This approach could reduce the need for invasive procedures such as digital subtraction angiography, leading to more efficient medical resource utilization and improved patient outcomes.

Characterizing diffusion from microdialysis catheters in the human brain: a magnetic resonance imaging study with gadobutrol.
Cerebral microdialysis (CMD) catheters allow continuous monitoring of patients' cerebral metabolism in severe traumatic brain injury (TBI). The catheters consist of a terminal semi-permeable membrane that is inserted into the brain's interstitium to allow perfusion fluid to equalize with the surrounding cerebral extracellular environment before being recovered through a central non-porous channel. However, it is unclear how far recovered fluid and suspended metabolites have diffused from within the brain, and therefore what volume or region of brain tissue the analyses of metabolism represent. ⋯ Cerebral microdialysis allows continuous monitoring of regional cerebral metabolism-the volume of which is now clearer from this study. It also has the potential to deliver small molecule therapies to focal pathologies of the human brain. This study provides a platform for future development of new catheters optimally designed to treat such conditions.

A Survey on Monitoring and Management of Cerebral Vasospasm and Delayed Cerebral Ischemia After Subarachnoid Hemorrhage: The Mantra Study.
Cerebral infarction from delayed cerebral ischemia (DCI) is a leading cause of poor neurological outcome after aneurysmal subarachnoid hemorrhage (aSAH). We performed an international clinical practice survey to identify monitoring and management strategies for cerebral vasospasm associated with DCI in aSAH patients requiring intensive care unit admission. ⋯ This survey demonstrated variability in monitoring and management strategies for DCI related to vasospasm after aSAH. These findings may be helpful in promoting educational programs and future research.
