To assess 2-year excess mortality or institutionalization risk associated with hip fracture in community-dwelling people aged 60 and over. ⋯ For both men and women who incur a hip fracture, the risks of dying or being institutionalized within 2 years are higher than for their peers. The independent effect of hip fracture on this outcome was significantly greater for men than women. The marked influences of prefracture health status, physical limitations, and gender on outcome have important implications for preventative strategies.

Implantable left ventricular assist devices (LVADs) have been used primarily as bridges to cardiac transplantation, although some patients have been maintained long term on these devices and a few have recovered enough to be weaned. This trial was designed to evaluate their suitability as long-term myocardial replacement therapy in patients who are ineligible for cardiac transplantation. ⋯ The use of an LVAD resulted in improved survival and quality of life in patients with extremely severe heart failure (75% 1-year mortality in the medical control group). However, only 23% of LVAD patients survived to 2 years and complications were frequent.

One-year survival and neurological outcome after pediatric cardiopulmonary resuscitation.
Reported survival after cardiopulmonary resuscitation (CPR) in children varies considerably. We aimed to identify predictors of 1-year survival and to assess long-term neurological status after in- or outpatient CPR. ⋯ Independent determinants of long-term survival of pediatric resuscitation are location of arrest, underlying cause, and duration of CPR. Long-term survivors have little or no change in neurological status.

[The influence of poor nutrition on the necessity of mechanical ventilation among children admitted to the Pediatric Intensive Care Unit].
To determine the relation between children's nutritional status when they are admitted to the Pediatric Intensive Care Unit, the necessity and length of mechanical ventilation and the mortality rate. ⋯ In our study, we could demonstrate that malnourishment in children under 4 years old admitted to the pediatric intensive care unit represented a decisive factor on evolution, increasing significantly the necessity and the length of mechanical ventilation as well as the length of stay at the pediatric intensive care unit.

Assessing the burden of disease and injury in Los Angeles County using disability-adjusted life years.
This study was designed to assess the burden of disease and injury in the Los Angeles County population using Disability-Adjusted Life Years (DALYs), a composite measure of premature mortality and disability that equates to years of healthy life lost. ⋯ The DALYs measure is a promising new tool to improve the capacity of local health departments and other health agencies to assess population health and establish an evidence base for public health decisions.
