Scientific progress in pathophysiology and differentiation of pain, functional diagnostic of pain and emerging treatments highlight this subject. Basics of development of pain, as well as differentiation of nociceptive and neuropathic pain are depicted; the latter is illustrated with the example of postherpetic neuralgia. Complex regional pain syndromes are described as a third pain complex. ⋯ Overviews amongst others from the Cochrane library for local anesthetics, opioids and for the treatment of postherpetic neuralgia are included. Tables are provided to simplify use in daily practice. The goal of this overview is a conceptual development of pain diagnosis and therapy in dermatology.

ReviewEpinephrine inhalers in emergency sets of patients with anaphylaxis.
Emergency sets are prescribed to allow patients with anaphylaxis to treat themselves before professional aid arrives. The need for epinephrine in this setting is well-accepted, but how it should be administered is still controversial. Epinephrine preparations can be administered orally, subcutaneously, intramuscularly or as aerosols. ⋯ The following review article summarizes data currently available on the use of epinephrine preparations in first-aid treatment of anaphylaxis. Studies have shown that the plasma concentrations needed for hemodynamic stabilization cannot be reached with epinephrine inhalers. Since most cases of hypotension in anaphylaxis cannot be effectively treated with epinephrine inhalers, the prescriber should be aware of this before including them in an emergency pack.

Though most dermatoses are not life-threatening, skin diseases play an important role in intensive care medicine. Skin findings in intensive care patients may reflect the underlying disease or be complications of intensive medical care. Most important are drug reactions, infections, bacterial toxin reactions, erythroderma, ANCA-positive vasculitides (such as Wegener granulomatosis) and bleeding disorders.

ReviewSkin cancer: follow-up, rehabilitation, palliative and supportive care.
Follow-up, rehabilitation and palliative/supportive care are essential parts of the care and treatment of skin cancer patients. This review provides an overview on the state of knowledge and recent developments in these three disciplines, hereby providing standard operating procedures for the dermatologist in the care of skin cancer patients. Especially in follow-up and supportive care, the results of recent clinical trials have led to significant changes in the standard of care. These new insights and their consequences are the special focus of this article.

ReviewAssessment of publications using methods of evidence-based medicine: an introduction using a dermatological clinical study.
Evidence-based medicine is the explicit use of the currently best available scientific data for decision-making in the management of individual patients, with guidelines being among the instruments for its implementation. Publication of the German S3 guideline on the treatment of plaque-type psoriasis is a mile-stone for German dermatology, as it is the first dermatologic guideline at this highest level of evidence. ⋯ This article explains the principles of such an analysis, so the reader can gain insight into the methodology of a crucial aspect of guideline generation. This information is of general relevance, as it provides help for every physician who wishes to critically read original scientific publications.
