Correlation between MRI findings of pituitary gland and prolactin level among hyperprolactinemia adult female Saudi patients in rural areas: A retrospective multicentric study.
Identifying the prolactin threshold that necessitates pituitary magnetic resonance imaging (MRI) in patients with hyperprolactinemia remains challenging. Therefore, developing standards for serum prolactin level criteria to predict prolactinoma is critical. This study aimed to investigate the correlation between hyperprolactinemia and the presence of pituitary adenoma among Saudi female patients with verified prolactin levels. ⋯ The ideal cutoff prolactin level for diagnosing pituitary adenoma was > 38.71 ng/mL with a sensitivity of 77.23% and specificity of 40.91%. It is prudent to perform pituitary imaging in most cases of hyperprolactinemia without event etiology, even if the condition is minor, due to the high prevalence of pituitary anomalies in female patients with hyperprolactinemia at serial sampling. A multidisciplinary strategy is necessary for a comprehensive diagnosis, treatment, and follow-up approach to improve the clinical outcomes of these individuals.

Diagnostic value of SAT-TB in smear-negative pulmonary tuberculosis: A diagnostic accuracy study.
This study aimed to evaluate the diagnostic value of rapid simultaneous RNA amplification and testing for tuberculosis (SAT-TB) in smear-negative pulmonary tuberculosis (PTB). We performed a multicenter prospective analysis of 206 patients with smear-negative suspected PTB between December 2018 and March 2022. We collected sputum or bronchoalveolar lavage fluid (BALF) for simultaneous SAT-TB and Xpert Mycobacterium tuberculosis/rifampin (MTB/RIF) assays. ⋯ The SAT-TB and Xpert MTB/RIF assays were highly consistent in diagnosing smear-negative PTB. It is a valuable method for early detection, prevention, and managing smear-negative PTB suspects. Meanwhile, the detection efficiency and cost-effectiveness of SAT-TB are more suitable for the rapid diagnosis of smear-negative PTB in low- and middle-income countries.

Relevance of oxidative stress for small intestinal injuries induced by nonsteroidal anti-inflammatory drugs: A multicenter prospective study.
Several reports revealed that oxidative stress was involved in the mouse model of nonsteroidal anti-inflammatory drug (NSAIDs)-induced small intestinal mucosal injuries. Thus, we aimed to investigate in the prospective clinical study, that the relevance of oxidative stress balance in small intestinal mucosal injury in NSAIDs users. We prospectively included 60 patients who had been taking NSAIDs continuously for more than 3 months and exhibited obscure gastrointestinal bleeding (number UMIN 000011775). ⋯ In aspirin users, d-ROM and OXY levels did not differ significantly with respect to mucosal injuries or ulcers. However, in nonaspirin users, OXY level was significantly lower in the mucosal injury group (P = .04), and d-ROM levels were significantly higher in the ulcer group (P = .02). Nonaspirin NSAIDs-induced intestinal mucosal injury is associated with antioxidant systems, resulting in increased oxidative stress.

Evaluation of risk factors for postpartum cerebral venous sinus thrombosis, a multicenter retrospective observational study.
The postpartum period is a well-defined risk factor for cerebral venous sinus thrombosis (CVST). However, it is unclear which patients are at risk for CVST in the postpartum period. Thus, determining some CVST risk factors in postpartum patients may be useful for preventing the disorder in this population. ⋯ Women with PE who had recently delivered were found to be at increased risk of developing CVST. The primary limitation of this study is that it was retrospective, and the control group was hospital-based. We recommend that these findings be confirmed by multicenter prospective international studies.

Association between oral loop diuretics duration before discharge and hospital readmission in acute decompensated heart failure: A multicenter retrospective cohort study.
There are uncertainties when to start patients on oral loop diuretics after managing acute decompensated heart failure (ADHF) before discharge. This study aims to investigate the impact of prolonging observation duration on hospital readmissions following the switch to oral loop diuretics before discharge in patients with ADHF. A multicenter retrospective study that included adult patients (>18 years) diagnosed with ADHF and discharged on oral loop diuretics in Saudi Arabia. ⋯ ADHF patients with <24 hours observation durations were significantly associated with higher all-cause 30-day readmissions compared to those with ≥24 hours observations (odds ratio = 1.83, 95% confidence interval = 1.11-3.02, P = .017). Our study showed a significant decrease in all-cause 30-day readmissions for ADHF patients who received oral loop diuretics at discharge with a longer duration of observation. This study emphasizes the significance of standardizing the observational period when initiating oral loop diuretics before discharge following admission to ADHF.
