Increasing evidence suggests that acute kidney injury (AKI) mediates a systemic response that can lead to multiple organ failure. AKI may manifest in a variety of clinical scenarios including kidney transplantation and is associated with a significantly high mortality. ⋯ Furthermore, it remains unclear as to whether cytokines mediate similar or differing responses in different end organs. This review summarizes the effects of AKI on remote organs and explores the role of systemic cytokines in mediating distant organ damage.

Incidence and mortality of acute kidney injury in acute myocardial infarction patients: a comparison between AKIN and RIFLE criteria.
Acute kidney injury (AKI) is associated with adverse outcomes after acute ST elevation myocardial infarction (STEMI). The recently proposed AKI network (AKIN) suggested modifications to the consensus classification system for AKI known as the risk, injury, failure, loss, end-stage (RIFLE) criteria. The aim of the current study was to compare the incidence and mortality (early and late) of AKI diagnosed by RIFLE and AKIN criteria in the STEMI patients undergoing primary percutaneous intervention (PCI). ⋯ AKIN criteria are more sensitive in defining AKI compared with the RIFLE criteria in STEMI. However, no difference exists in the mortality risk provided by these two scoring systems.

Comparison between the effects of intraoperative human albumin and normal saline on early graft function in renal transplantation.
The aim of the current study was to compare the effects of intraoperative infusion of 20 % human albumin versus 0.9 % normal saline on early and late graft function in renal transplantation. ⋯ This study revealed that the use of 20 % human albumin as an intraoperative volume expander provides no more benefit than the use of 0.9 % normal saline in terms of immediate graft function in living donor renal transplantation.

Traumatic testicular dislocation is a rare entity. It occurs after a direct blunt scrotal trauma causing the testicle to migrate outside the scrotum, most frequently to the superficial inguinal region. ⋯ Traumatic testicular dislocation is rare and diagnosis can be elusive. It should be suspected in motorcycle and high-energy accidents around the groin area and depends on a careful physical examination. With proper management, prognosis is excellent.

To evaluate our clinical outcomes in managing acute kidney injury (AKI) resulting from ceftriaxone-induced urolithiasis with emergency treatment. ⋯ Ceftriaxone could result in urolithiasis in children, which could also cause AKI. Appropriate and timely surgical management by conventional treatments will mostly lead to full recovery of renal functions.
