Role of femoral head material on readmission and mortality rates following elective primary total hip arthroplasty in Medicare patients.
The role of different femoral head materials for total hip arthroplasty (THA) has been widely studied in the context of wear properties and corrosion. Cobalt chrome (CoCr) femoral heads are commonly used as a standard of comparison to other materials such as ceramic and oxidized zirconium (OxZi). This study aims to evaluate the impact of femoral head material on clinical outcomes in elective primary THA patients. ⋯ Retrospective analysis.

Quantitative computed tomography has higher sensitivity detecting critical bone mineral density compared to dual-energy X-ray absorptiometry in postmenopausal women and elderly men with osteoporotic fractures: a real-life study.
Dual-energy X-ray absorptiometry (DXA) is considered the gold standard for the diagnosis of osteoporosis and assessment of fracture risk despite proven limitations. Quantitative computed tomography (QCT) is regarded as a sensitive method for diagnosis and follow-up. Pathologic fractures are classified as the main clinical manifestation of osteoporosis. The objective of the study was to compare DXA and QCT to determine their sensitivity and discriminatory power. ⋯ Our results support that volumetric measurements by QCT in preselected subjects represent a more sensitive method for the diagnosis of osteoporosis and prediction of fractures compared to DXA.

PCL preservation or sacrifice does not influence clinical outcomes and survivorship at mid-term follow-up of a J-curve CR total knee replacement with a medial congruent liner and a functional coronal alignment.
In recent years, increasing interest has arisen for medial pivoting TKA implants, designed to mimic the physiological knee kinematics, maximizing the contact area on the medial compartment of the knee, increasing anterior-posterior stability, and contributing to a ball-in-socket effect that allows the posterior cruciate sacrifice without a post-CAM mechanism. The medial congruent liner works with a J-curve cruciate-retaining (CR) femur without a real ball-in-socket design. The aim of this study was to evaluate the clinical outcomes and survival of these implants, with or without PCL sparing, at a medium follow-up. ⋯ TKA with a medially congruent insert, showed promising results at mid-term follow-up, PCL preservation or sacrifice did not affect the clinical outcomes and survival. Further follow-up will be needed to evaluate these results at long term.

Complications following single-stage bilateral total knee arthroplasty and unilateral procedures: experience of a high-volume community hospital.
The safety of single-stage bilateral total knee arthroplasty (SSBTKA) compared to unilateral total knee arthroplasty (TKA) remains controversial. The present study compares the 90-day postoperative complications encountered following SSBTKA and unilateral TKA in an unselected cohort of patients performed at a high-volume community hospital. ⋯ SSBTKA did not result in greater complications when compared to unilateral TKA in this particular cohort. As expected, transfusion rates will likely be higher and there will be a greater need for acute inpatient care following surgery for SSBTKA patients.
