To evaluate the effect of Medicare Shared Savings Program accountable care organizations (ACOs) on hospital readmission after common surgical procedures. ⋯ Readmissions following common procedures decreased significantly from 2010 to 2014. Hospital affiliation with Shared Savings ACOs was associated with significant additional reductions in readmissions. This emphasis on readmission reduction is 1 mechanism through which ACOs improve value in a surgical population.

To examine long-term survival and causes of death among working-age patients with acute pancreatitis (AP) compared with the normal population. ⋯ The long-term mortality among patients admitted to the hospital due to (mainly alcohol induced) AP was 4 times higher than that in the age- and sex-matched control population. The significant difference in the causes of death between patients with alcohol AP and controls could be explained by alcohol-related diseases. Occurrences of AP without an alcohol etiology had a minimal impact on survival.

We aimed to evaluate the use of miR-200b as a prenatal transplacental therapy in the nitrofen rat model of abnormal lung development and congenital diaphragmatic hernia (CDH). ⋯ Our data indicate that miR-200b improves PH and decreases the incidence of CDH. Future studies will further exploit this newly discovered prenatal therapy for lung hypoplasia and CDH.

To characterize patterns of engagement in work during the 4-year period after major traumatic injury, and to identify factors associated with those patterns. ⋯ A range of personal, occupational, injury, health, and compensation system factors influence RTW patterns after serious injury. Early identification of people at risk for delayed, failed, or no RTW is needed so that targeted interventions can be delivered.

BACKGROUND:: Breath VOCs have the potential to noninvasively diagnose cancer. ⋯ Exhaled breath samples were collected using 2-L double-layered Nalophan bags, and were analyzed using selected-ion-flow-tube mass-spectrometry. Gold-standard test for comparison was endoscopy for luminal inspection and computed tomography (CT) to confirm cancer recurrence. Three studies were conducted: RESULTS:: CONCLUSION:: This study suggests the association of a single breath biomarker with the primary presence and recurrence of CRCa. Further multicenter validation studies are required to validate these findings.
