Surgical management of parasagittal meningiomas (PMs) still remains controversial in literature. The need to pursue a resection as radical as possible along with the high risk of venous injuries contribute to making the sinus opening a widely argued choice. The aim of our study was to analyze factors affecting the risk of recurrence and to assess clinical outcome of patients that underwent surgical resection of PMs with conservative or aggressive management of the intrasinusal portion. ⋯ Removal of the intrasinusal portion of the tumor using standard procedures is not associated with a higher risk of permanent deficit or worsening of the KPS, while reducing the risk of recurrence.

Patients with Lumbar Spinal Stenosis (LSS) typically complain of back pain and leg pain. These symptoms reduce the quality of life and also cause sleep disturbances. This study compares pregabalin and limaprost's efficacy in LSS for pain, disability, quality of life, and sleep, aiming to offer insights for medication selection. ⋯ Efficacy of pregabalin, limaprost in back and leg pain, ODI, EQ-5D, and sleep quality, but there was no significant difference between the two groups. Thus, it is advisable to prescribe based on individual drug responses and potential complications.

To propose a novel surgical strategy-thoracic anterior controllable ante-displacement fusion (TACAF) to treat mT-OPLL, and investigated its safety and efficacy. ⋯ This strategy provides a novel solution for the treatment of mT-OPLL with favorable recovery of neurological function, the tension of spinal cord, and less complications.

Diagnosing early lumbar spondylolisthesis is challenging for many doctors because of the lack of obvious symptoms. Using deep learning (DL) models to improve the accuracy of X-ray diagnoses can effectively reduce missed and misdiagnoses in clinical practice. ⋯ Our Two-stage deep learning model provides doctors with a reference basis for the better diagnosis and treatment of early lumbar spondylolisthesis.

Intramedullary spinal cord tumors (IMSCTs) are a rare subgroup of neoplasms, encompassing both benign, slow-growing masses and malignant lesions; radical surgical excision represents the cornerstone of treatment for such pathologies regardless of histopathology, which, on the other hand, is a known predictor of survival and neurological outcome post-surgery. The present study aims to investigate the relevance of other factors in predicting survival and long-term functional outcome. ⋯ Consistently with literature, preoperative neurological function is the most important factor predicting long-term functional outcome (0.17, CI 0.069-0.57 with p=0.0018), advocating for early surgery in the management of IMSCTs, whereas late complications such as myelopathy and neuropathic pain where present regardless of preoperative function.
