The current investigation was aimed on the effect of topical lidocaine application on the wound after back surgery and the emergence from general anesthesia in the rat. ⋯ Topical application of local anesthetics prolonged the time to extubation without the delay for an appearance of eyelid reflex. The developed method using rats could be useful for future investigations of the nature of emergence from general anesthesia.

Olprinone hydrochloride (OLP) is a new phosphodiesterase III inhibitor with positive inotropic and vasodilator properties. It is characterized by strong inotropic effect and relatively weak vasodilating effect. Although the method for administration of OLP has been reasonably optimized, no studies have examined the administration mode for patients with off-pump coronary artery bypass grafting (OPCAB). ⋯ The study showed that OLP administrated during OPCAB may have improved hemodynamics in patients for OPCAB.

Case Reports
[Postponed or canceled drug challenge tests and side effects of the test drug--a report of four cases].
Drug challenge test (DCT) is performed to evaluate chronic pain pharmacologically and determine its medical treatment. One test drug is administered in one day for DCT and characterization of the test drug. Four patients developed side effects of the test drugs for DCT in whom other drug tests were postponed or canceled. ⋯ A 32-year-old woman with chronic pelvic pain had skin eruption on her thigh the day after phentolamine test. Although the amount of the test drug in DCT is small and its half-life is short, long-term side effects might occur. We should decrease the amounts or frequencies of ketamine and morphine, and administer them taking long intervals before other tests.

The clinical effect of continuous infusion of landiolol with buprenorphine and lidocaine was evaluated for its effects an attenuating the cardiovascular responses to endotracheal intubation. ⋯ Continuous infusion of landiolol 0.04 mg x kg(-1) x h(-1) with buprenorphine and lidocaine can completely attenuate the hemodynamic response to tracheal intubation.

Liver dysfunction has major impacts on the pharmacokinetics and pharmacodynamics of anesthetics. This study was designed to evaluate propofol concentrations during and at the end of total intravenous anesthesia (TIVA) for hepatectomy. ⋯ Recovery from TIVA is delayed in hepatectomy patients. We speculate that this is attributable to altered pharmacodynamics in these patients.
