Review[Hemodynamic impact of positive end expiratory pressure (PEEP) during severe respiratory failure: present view].
Bedside evaluation of pulmonary mechanics and thoracic computed axial tomography have changed the ventilatory management of patients suffering an acute respiratory failure caused by adult respiratory distress syndrome (ARDS). Mortality has been reduced limiting tidal volumes, which avoids alveolar overdistention and by the use of positive end expiratory pressure (PEEP), that reduces the damage caused by cyclical alveolar collapse-reopening. ⋯ Usually, it is not easy to achieve an equilibrium between an optimal lung recruitment without producing alveolar overdistention and hemodynamic adverse effects such as hypotension and reduction of cardiac output. This paper reviews the interactions between heart and lung.

Review[Febrile neutropenia in the child with cancer. Current concepts about risk assessment and selective management].
The risk for invasive bacterial infection (IBI) in cancer pediatric patients with febrile neutropenia, is variable. Clinicians worldwide are increasingly considering selective strategies for children at low risk for IBI, including shortened antimicrobial course, early hospital discharge, oral antimicrobial treatment, and management as outpatients. These strategies would significantly benefit these children and health care systems. ⋯ During 1996-1997 we identified risk factors of IBI in a group of 447 febrile neutropenic episodes. During 1999-2000 we validated these risk factors in a prospective study that included 263 febrile neutropenic episodes. A model of risk prediction was developed and is currently being evaluated for the selection of low risk patients who are treated as inpatients for 24 hours, followed by outpatient treatment.

Review Case Reports[Gemcitabine in the treatment of 4 patients with cholangiocarcinoma].
Cholangiocarcinoma is a biliary tree cancer of unknown etiology, whose symptoms are unspecific and is usually detected in advanced stages. Surgery continues to be the only curative therapy. Median survival in patients with inoperable tumors ranges between 5 and 8 months. ⋯ We report four patients with advanced cholangiocarcinoma, treated with gemcitabine 1000 mg/m2, weekly for 3 weeks every 28 days. There was a stabilization of tumor size and symptoms were alleviated. Toxicity was low and there was a probable prolongation of survival.

Review[Prevention and treatment of obesity since childhood: strategy to decrease the non transmissible chronic diseases in adult].
The prevalence of obesity among children and teenagers is increasing by 1.5% per year, probably due to a higher consumption of highly caloric foods and to physical inactivity. Hypercholesterolemia, increased insulin levels and high blood pressure of childhood obesity, precede atherosclerosis, coronary artery disease, diabetes and hypertension in adulthood. ⋯ They aim at a change in dietary habits and increasing physical activity. A well balanced healthy diet and a decrease in physical inactivity time will result in a successful treatment approach for obesity.

Review[Inactivation of tumor suppressor genes in uterine cervix carcinogenesis].
The importance of inactivation of tumor suppressor genes in the development/progression of carcinomas of the uterine cervix is reviewed. It is well known that HPV-related oncogenes are strongly linked to cervical cancer. However, fewer studies have explored the occurrence of inactivation of tumor suppressor genes in this neoplasia. ⋯ Deletions are detected even in precursor lesions (cervical intraepithelial neoplasia, CIN). Some deletions have been correlated with prognostic parameters, such as stage, depth of invasion, and vascular space involvement. It is concluded that cervical carcinogenesis, like in other tumors, is a multistep process, characterized by the accumulation of events including activation of oncogenes, as well as inactivation of tumor suppressor genes.
