Multicenter StudyImpact of prevalent fractures on quality of life: baseline results from the global longitudinal study of osteoporosis in women.
To examine several dimensions of health-related quality of life (HRQL) in postmenopausal women who report previous fractures, and to provide perspective by comparing these findings with those in other chronic conditions (diabetes, arthritis, lung disease). ⋯ Previous fractures at a variety of bone locations, particularly spine, hip, and upper leg, or involving more than 1 location are associated with significant reductions in quality of life.

Randomized Controlled Trial Multicenter Study Comparative StudyA phase 3, double-blind, randomized, placebo-controlled study of armodafinil for excessive sleepiness associated with jet lag disorder.
To assess the effect of armodafinil, the longer-lasting isomer of modafinil, on jet lag disorder. ⋯ Armodafinil increased wakefulness after eastward travel through 6 time zones.

Multicenter Study Comparative StudyHypoglycemia and outcome in critically ill patients.
To determine whether mild or moderate hypoglycemia that occurs in critically ill patients is independently associated with an increased risk of death. ⋯ In critically ill patients, an association exists between even mild or moderate hypoglycemia and mortality. Even after adjustment for insulin therapy or timing of hypoglycemic episode, the more severe the hypoglycemia, the greater the risk of death.

Randomized Controlled Trial Multicenter StudyArmodafinil for treatment of excessive sleepiness associated with shift work disorder: a randomized controlled study.
To assess the effect of armodafinil, 150 mg, on the physiologic propensity for sleep and cognitive performance during usual night shift hours in patients with excessive sleepiness associated with chronic (> or =3 months) shift work disorder (SWD) of moderate or greater severity. ⋯ In patients with excessive sleepiness associated with chronic SWD of moderate or greater severity, armodafinil significantly improved wakefulness during scheduled night work, raising mean nighttime sleep latency above the level considered to indicate severe sleepiness during the daytime. Armodafinil also significantly improved measures of overall clinical condition, long-term memory, and attention.

Multicenter Study Comparative StudyHyperglycemia in patients undergoing cerebral aneurysm surgery: its association with long-term gross neurologic and neuropsychological function.
To evaluate whether elevated intraoperative blood glucose concentrations are associated with an increased risk of long-term neurologic dysfunction in patients at risk for ischemic brain injury. ⋯ In patients at high risk for ischemic brain injury, intraoperative hyperglycemia, of a magnitude commonly encountered clinically, was associated with long-term changes in cognition and gross neurologic function.
