Meta Analysis Comparative StudyMeta-analysis of randomized controlled trials of vancomycin for the treatment of patients with gram-positive infections: focus on the study design.
To study the effectiveness and safety of vancomycin compared with that of other antibiotics for the treatment of gram-positive infections. ⋯ On the basis mainly of data from open-label trials, vancomycin is a treatment choice that is as effective as other available antibiotics for patients with gram-positive infections. Study design seems to make a major contribution to the outcome.

Review Meta AnalysisSexual abuse and lifetime diagnosis of psychiatric disorders: systematic review and meta-analysis.
To systematically assess the evidence for an association between sexual abuse and a lifetime diagnosis of psychiatric disorders. ⋯ A history of sexual abuse is associated with an increased risk of a lifetime diagnosis of multiple psychiatric disorders.

Review Meta AnalysisIntraoperative echocardiography in valvular heart disease: an evidence-based appraisal.
Intraoperative (IO) transesophageal echocardiography (TEE) is widely used for assessing the results of valvular heart disease (VHD) surgery. Epiaortic ultrasonography (EAU) has been recommended for prevention of perioperative strokes. To what extent does high-quality evidence justify the widespread use of these imaging modalities? In March 2009, we searched MEDLINE (PubMed and OVID interfaces) and EMBASE for studies published in English using database-specific controlled vocabulary describing the concepts of IOTEE, cardiac surgery, VHD, and EAU. ⋯ The value of IOTEE in isolated aortic valve replacement remains less clear. Evidence supporting EAU is scientifically more robust but conflicting. These findings have important clinical policy and research implications.

Review Meta AnalysisUse of drug-eluting stents in patients with coronary artery disease and renal insufficiency.
Renal insufficiency (RI) has been shown to be associated with increased major adverse cardiovascular events after percutaneous coronary intervention. We reviewed the impact of RI on the pathogenesis of coronary artery disease and outcomes after percutaneous coronary intervention in the form of drug-eluting stent (DES) implantation in these high-risk patients. We searched the English-language literature indexed in MEDLINE, Scopus, and EBSCO Host research databases from 1990 through January 2009, using as search terms coronary revascularization, drug-eluting stent, and renal insufficiency. ⋯ The outcomes after coronary revascularization were less favorable in patients with RI than in those with normal renal function. In patients with RI, DES implantation yielded better outcomes than did use of bare-metal stents. Randomized trials are needed to define optimal treatment of these high-risk patients with coronary artery disease.

Meta AnalysisBenefits and risks of long-term amiodarone therapy for persistent atrial fibrillation: a meta-analysis.
To assess whether amiodarone, as part of a strategy to achieve sinus rhythm, is safe and effective compared with a placebo or rate control drug in patients with persistent atrial fibrillation (AF) of more than 30 days' duration. ⋯ Amiodarone, as part of a strategy to achieve and maintain sinus rhythm, appears to be safe and effective in patients with persistent AF. However, some patients may not tolerate the adverse effects of this agent.
