The use of apnea test and brain death determination in patients on extracorporeal membrane oxygenation: A systematic review.
To review practices of brain death (BD) determination in patients on extracorporeal membrane oxygenation (ECMO). ⋯ Performing AT for patients with ECMO was associated with high failure rate and hemodynamic complications. Our study highlights the variability in practice in regard to the AT and supports the use of ancillary tests to determine BD in patients on ECMO.

Comparative Study Observational StudyOxygen Reserve Index: Utility as an Early Warning for Desaturation in High-Risk Surgical Patients.
Perioperative pulse oximetry hemoglobin saturation (Spo2) measurement is associated with fewer desaturation and hypoxia episodes. However, the sigmoidal nature of oxygen-hemoglobin dissociation limits the accuracy of estimation of the partial pressure of oxygen (Pao2) >80 mm Hg and correspondingly limits the ability to identify when Pao2 >80 mm Hg but falling. We hypothesized that a proxy measurement for oxygen saturation (Oxygen Reserve Index [ORI]) derived from multiwavelength pulse oximetry may allow additional warning time before critical desaturation or hypoxia. To test our hypothesis, we used a Masimo multiwavelength pulse oximeter to compare ORI and Spo2 warning times during apnea in high-risk surgical patients undergoing cardiac surgery. ⋯ In adult high-risk surgical patients, ORI provided clinically relevant added warning time of impending desaturation compared to Spo2. This additional time may allow modification of airway management, earlier calls for help, or assistance from other providers. The potential patient safety impact of such monitoring requires further study.

Determination of Brain Death/Death by Neurologic Criteria: The World Brain Death Project.
There are inconsistencies in concept, criteria, practice, and documentation of brain death/death by neurologic criteria (BD/DNC) both internationally and within countries. ⋯ This report provides recommendations for the minimum clinical standards for determination of brain death/death by neurologic criteria in adults and children with clear guidance for various clinical circumstances. The recommendations have widespread international society endorsement and can serve to guide professional societies and countries in the revision or development of protocols and procedures for determination of brain death/death by neurologic criteria, leading to greater consistency within and between countries.

Multicenter Study Observational StudyCharacterization of respiratory compromise and the potential clinical utility of capnography in the post-anesthesia care unit: a blinded observational trial.
The utility of capnography to detect early respiratory compromise in surgical patients after anesthesia is unclear due to limited prospective data. The purpose of this trial was to determine the frequency and duration of capnography-detected respiratory adverse events in the post-anesthesia care unit (PACU). In this prospective observational trial, 250 consenting patients undergoing elective surgery with general anesthesia were monitored by standard monitoring together with blinded capnography and pulse oximetry monitoring. ⋯ Three patients' blinded capnography was unblinded to facilitate clinical care. Respiratory adverse events are frequent in the PACU, and the addition of capnography and IPI to current standard monitoring provides potentially clinically relevant information on respiratory status, including early warning of some respiratory adverse events. The use of extracorporeal membrane oxygenation (ECMO) in adult patients continues to increase. Suspicion of brain death while on ECMO creates a conundrum. The American Academy of Neurology states that apnea testing is a critical component of the process to declare brain death. However, there is a paucity of literature on apnea testing for confirmation of brain death in patients on venoarterial ECMO and venovenous ECMO. Traditional apnea testing does not consider ECMO physiology or de-recruitment of the lungs in this subset of critically ill patients. Complications with traditional apnea testing include hemodynamic instability that may lead to cardiac arrest and death. ⋯ In 5 subjects on ECMO, the carbogen method for apnea testing as part of the process to declare brain death was accurate in predicting the end point of the apnea test. With the increased use of ECMO in adults and the ongoing need for organs, methods to confirm brain death with apnea testing while on ECMO should be further studied.
