There is a high level of evidence that a short time to surgery (TTS) improves the outcome for patients with hip fractures. Accordingly, recommendations for timely treatment have been included in national guidelines. As patient characteristics appear to be similar, it seems reasonable that these guidelines are applicable to other fracture entities, such as knee periprosthetic femoral fracture (PPF). This monocentric retrospective study aimed to investigate outcome-related risk factors, particularly TTS, for knee PPF. ⋯ 1-year mortality after knee PPF was 8.3 %. With a high complication rate in the treatment of knee PPF, TTS was identified as a risk factor for general complications. Early treatment appears to be beneficial for patients with knee PPF.

Editor-We congratulate Lyne and colleagues1 on their initiative to explore and improve consent around perioperative mortality in children. Providing such data for families and clinicians is a key purpose of the Royal College of Anaesthetists' National Audit Projects (NAPs), and to this end, NAP7 studied perioperative cardiac arrest.2-4.

Observational StudyDevelopment of a Scoring Model to Predict Severe Dengue in Children at Admission in the Emergency Care: An Observational Study.
Dengue has emerged as the most widely spread mosquito-borne disease, hyperendemic in India. Although severe dengue occurs only in a small proportion of cases, delays in the recognition and management can result in significant morbidity and mortality. Risk stratification of children in the emergency care and identifying those at risk for worsening during hospital stay facilitates optimum utilization of health care resources. The objective of our study was to develop and validate a scoring model to predict the development of severe dengue in hospitalized children by identifying risk factors present in them at the time of admission to the emergency department. ⋯ The dengue severity scoring model was found to have reasonable diagnostic accuracy in predicting severe disease prior to hospitalization. However, more studies are required to validate the score in different emergency care settings with varied pediatric populations.

Long working hours and cardiovascular disease mortality: Prospective evidence from the United States.
Cardiovascular disease (CVD) is the leading cause of death in the United States (U.S.). This study aimed to explore prospective associations between long working hours with CVD mortality using a large, national study in the U.S. ⋯ Long working hours are a significant risk factor for CVD mortality in this national sample of U.S. workers, and participants with low socioeconomic status are more vulnerable to the effects of long working hours on CVD deaths. These findings highlight the need for considering working hour interventions in public health strategies to improve cardiovascular health outcomes in the workforce.

Background: Acute lung injury (ALI) is a severe complication of sepsis, characterized by inflammation, edema, and injury to alveolar cells, leading to high mortality rates. Septic ALI is a complex disease involving multiple factors and signaling pathways. STEAP family member 1 (STEAP1) has been reported to be upregulated in a sepsis-induced ALI model. ⋯ Moreover, METTL14 silencing attenuated LPS-induced effects by decreasing STEAP1 expression in HPMECs, and STEAP1 silencing ameliorated cecal ligation and puncture-induced lung injury of mice. Conclusion: METTL14/IGF2BP2-mediated m6A modification of STEAP1 aggravated ALI induced by sepsis. These findings suggest potential therapeutic targets for the treatment of this disease.
