Randomized Controlled Trial Multicenter StudyEfficacy and safety of continuous infusion of fentanyl for pain control in preterm newborns on mechanical ventilation.
To evaluate the analgesic superiority and the safety equivalence of continuous fentanyl infusions versus fentanyl boluses in preterm infants on mechanical ventilation. ⋯ In very preterm infants on mechanical ventilation, continuous fentanyl infusion plus open-label boluses of fentanyl does not reduce prolonged pain, but does reduce acute pain and increase side effects compared with open-label boluses of fentanyl alone.

Multicenter StudyImpact of gender on sepsis mortality and severity of illness for prepubertal and postpubertal children.
To investigate differences in sepsis mortality between prepubertal and postpubertal males and females. ⋯ Sepsis mortality is similar in prepubertal males and females. However, postpubertal males have a higher sepsis mortality than postpubertal females, likely related to their greater severity of illness on PICU admission. These outcome differences in postpubertal children may reflect a hormonal influence on the response to infection or differences in underlying comorbidities, source of infection, or behavior.

Multicenter StudyBereaved parents' intentions and suggestions about research autopsies in children with lethal brain tumors.
To determine bereaved parents' perceptions about participating in autopsy-related research and to elucidate their suggestions about how to improve the process. ⋯ Because autopsy can help advance scientific understanding of the disease itself and because parents reported having no regret and even cited benefits, researchers should be encouraged to continue autopsy-related research. Parental perceptions about such studies should be evaluated in other types of pediatric diseases.

Randomized Controlled Trial Multicenter StudyThe NOFLO trial: low-flow nasal prongs therapy in weaning nasal continuous positive airway pressure in preterm infants.
To determine if low-flow nasal prongs therapy with room air, compared with no treatment, facilitates weaning from nasal continuous positive airway pressure (NCPAP) in very low birth weight (VLBW, birth weight <1500 g) infants. ⋯ In this study, we did not demonstrate a benefit of low-flow room air via nasal prongs to wean VLBW infants from NCPAP.

Multicenter StudyVariation in resource utilization across a national sample of pediatric emergency departments.
To describe variations in emergency department (ED) quality measures and determine the association between ED costs and outcomes for 3 pediatric conditions: asthma, gastroenteritis, and simple febrile seizure. ⋯ We observed variation in quality measures for patients presenting to pediatric EDs with common conditions. Higher costs were not associated with lower hospitalization or ED revisit rates.
