[Preoperative testing routines for healthy, asymptomatic patients in the Canary Islands (Spain)].
To analyze patterns of routine testing before elective/scheduled surgery in healthy/asymptomatic patients classified as ASA I or II according to the American Society of Anesthesiologists. ⋯ Hospitals and individual anesthesiologists differ considerably in how they request preoperative tests. The variations can not be explained solely by differing patient needs given that respondents were contemplating only healthy/asymptomatic individuals undergoing relatively simple procedures.

[Changes in the bispectral index and the spectral edge frequency 90 during different phases of anesthesia with sevoflurane].
Electroencephalographic monitoring is one of the techniques used to measure hypnosis during anesthesia. Efforts to find a means to apply this monitoring function are justified by cases of intraoperative awakening and are in the interest of guaranteeing patient welfare and controlling anesthesia. ⋯ Electroencephalographic monitoring is useful for distinguishing between states of consciousness and unconsciousness during anesthesia, and is valid for the phases of induction and recovery. BIS values over 58 and SEF90 values over 13 may indicate inadequate hypnosis.

Case Reports[Cauda equina syndrome after intradural anesthesia with bupivacaine].
A 70-year-old man developed pain and functional weakness in the lower limbs with dysesthesia and urinary retention after subarachnoid anesthesia with 0.5% hyperbaric bupivacaine. Neurological and electrophysiological tests and follow-up, as well as diagnostic images (CAT and MR), ruled out spinal cord lesions. ⋯ Associated with use of microcatheters for continuous subarachnoid anesthesia and 5% hyperbaric lidocaine, cauda equina syndrome is rare after a single injection of bupivacaine. Although the pathogenesis of cauda equina syndrome is poorly understood, there is agreement on the neurotoxicity of local anesthetics, particularly of 5% hyperbaric lidocaine.

Case Reports[Anesthetic management for cesarean delivery in a pregnant patient with congenital antithrombin III deficiency and pulmonary thromboembolism].
Antithrombin III (AT III) deficiency is a rare hereditary disease that predisposes a patient to thromboembolic complications. Anticoagulation is essential for preventing recurrence of thrombi. ⋯ This paper describes a woman with congenital AT III deficiency and heparin-treated pulmonary embolism whose pregnancy of 29 weeks had to be terminated by cesarean section upon signs of fetal distress. We discuss the pathophysiology and treatment in such cases.

Randomized Controlled Trial Clinical Trial[The effect of encephalogram bispectral index monitoring during total intravenous anesthesia with propofol in outpatient surgery].
To assess the effect of monitoring the encephalogram bispectral index (BIS) during outpatient surgery. Outcome measures were amount of propofol administered, awakening and discharge. ⋯ BIS monitoring allows for propofol titration that leads to a mean reduction of 32.6% in consumption, shortening the time until eye opening without causing intraoperative awareness or reducing patient satisfaction.
