The importance of medial compartment stability is recognized in total knee arthroplasty (TKA). To manage the medial extension-flexion gap, the posterior reference position can be changed from conventional posterior center to posterior medial in measured resection techniques. This study aimed to compare the intraoperative soft tissue balance and rotational kinematics between the posterior medial and posterior center reference groups. ⋯ The posterior reference position affects intraoperative kinematics and soft tissue balance in navigated PS TKA. Posterior medial reference PS TKA decreases the excessive tibial external rotation during midflexion and increases the flexion stability compared with conventional posterior center reference PS TKA.

When and how to operate the posterior malleolus fragment in trimalleolar fractures: a systematic literature review.
Whether or not and how to fixate the posterior malleolus fracture seems to depend on the fracture fragment size and its amount of dislocation, but clear guidelines for daily practice are lacking. In this review, we summarize the literature on preferred treatment of the posterior fragment in trimalleolar fractures. ⋯ Therapeutic level II.

Difficulties in Treating Complex Knee Injuries with Fracture of Posterior Tibial Plateau.
The aim of the study was to assess the outcomes of surgical treatment of complex knee injuries with a posterior inverted-L approach to the knee joint. ⋯ 1. Anatomical reduction and stable fixation of posterior plateau fractures of the proximal tibia are crucial in fracture management. 2. A posterior inverted-L approach is very useful when treating posterior tibial plateau fractures.
