Pain is the most common reason for visiting the Emergency Department (ED), and pain management is an important aspect of emergency care. Pain management might begin before emergency department arrival, by a patient's self-administered medications or alternative therapies. ⋯ The rate of self-administered intervention for pain relief before ED arrival was high. ED nurses have to take these interventions into account while performing pain assessment. The information may help to achieve better pain management in the ED.

Causes of excessive late death after trauma compared with a matched control cohort.
Studies on mortality following trauma have been restricted mainly to in-hospital or 30-day death. Mortality risk may be sustained several years after trauma, but the causes of late death have not been elucidated. The aim was to investigate mortality and analyse causes of late death after trauma. ⋯ Postinjury mortality is increased for several years after trauma. Excess mortality is largely attributed to recurrent trauma and other external causes of death.

Impact of acute kidney injury on patient outcome in out-of-hospital cardiac arrest: a prospective observational study.
Kidney disease after out-of-hospital cardiac arrest (OHCA) is incompletely described. We examined the occurrence of acute kidney injury (AKI) in OHCA patients and impact of AKI, with or without renal replacement therapy (RRT), on 6-month mortality and neurological outcome. ⋯ Kidney disease occurred in about half of patients successfully resuscitated from OHCA. Presence of AKI, but not RRT, was associated with unfavourable 6-month outcome.

Case ReportsBilateral Subpectoral Interfascial Plane Catheters for Analgesia for Sternal Fractures: A Case Report.
Sternal fractures occur after deceleration injuries such as falls and road traffic accidents. Recovery from isolated fractures is excellent, but mortality increases dramatically with concurrent chest injuries such as rib fractures and soft tissue injuries. Short-term complications include chest pain, which prevents patients from taking deep breaths and coughing, thereby predisposing them to chest infections. ⋯ The patient was successfully treated with a continuous infusion of local anesthetic into the subpectoral interfascial plane. We also discuss the use and potential benefits of the subpectoral interfascial plane block in the treatment of pain from sternal fractures.

Comparison of 2 available methods with Bland-Altman analysis for measuring intracompartmental pressure.
Acute compartment syndrome (ACS) is the result of increased intracompartmental pressure (ICP), and to avoid a delay in diagnosis requires ICP measurement. This study was designed to compare 2 available methods with Bland-Altman analysis for measuring ICP in experimental animal models, healthy volunteers, and patients with suspected ACS to evaluate their agreement and interchangeability. ⋯ The invasive pressure monitoring via IABPMS can be used as an alternative to the Whitesides method, thanks to the sufficient agreement between the 2 methods in ICP measurement, and also for its advantages recommended as a novel diagnostic approach to ACS in experimental and clinical applications.
