Meta AnalysisEffects of Garlic Extract on Lipid Profile in Patients with Coronary Artery Disease: A Systematic Review and Meta-Analysis of Randomized Clinical Trials.
This meta-analysis was conducted to evaluate the effects of garlic extract on total cholesterol (TC), triglycerides (TG), low-density lipoprotein-cholesterol (LDL-c) and high-density lipoprotein-cholesterol (HDL-c), among the patients with coronary artery disease (CAD). Literature searches were conducted in EMBASE, Scopus, PubMed, Web of Science and Cochrane Library until Sep18th, 2020. Inter-study heterogeneity was examined using Cochrane's Q and I2 tests. ⋯ Significant heterogeneity was observed for HDL-c (I2 = 76.8%). However, the findings of sensitivity analysis revealed that upon exclusion of the potential heterogeneity source, the pooled WMD on HDL-c levels were stable. Garlic supplementation may result in a decrease in TC, but will not affect TG, HDL-c and LDL-c levels among CAD patients.

Randomized Controlled TrialThe effect of low dose ketamine on postoperative quality of recovery in patients undergoing breast cancer surgery: a randomized, placebo-controlled trial.
Low-dose ketamine has been proved to reduce opioid consumption, prevent depressant action and improve postoperative analgesia. Women undergoing mastectomy experience may not only have persistent postoperative pain syndromes but also emotional problems. However, the effect of intraoperative infusion of low-dose ketamine on postoperative quality of recovery among these patients has not yet been fully studied. ⋯ Intraoperative low-dose ketamine infusion did not improve the overall quality of recovery on POD 1 in patients undergoing breast cancer surgery.

ReviewA systematic review of cases of West African Crystalline Maculopathy.
West African crystalline maculopathy (WACM) is, reportedly, a rare condition whose aetiology remains unclear. This study aims to describe the epidemiology, summarizing the identified risk factors and clinical characteristics of cases of WACM, with the goal of highlighting presentation patterns and the clinical course of the condition. ⋯ West African crystalline retinopathy is a seemingly innocuous condition affecting black or African people, who were also born in Africa. The majority of the patients have a vascular retinopathy. Longitudinal studies, particularly in Africa, may be required to elucidate the aetiology, as well as the long-term prognosis, of the crystals.

ReviewSevere and life-threatening COVID-19-related mucocutaneous eruptions: A systematic review.
Earlier diagnosis and the best management of virus-related, drug-related or mixed severe potentially life-threatening mucocutaneous reactions of COVID-19 patients are of great concern. These patients, especially hospitalised cases, are usually in a complicated situation (because of multi-organ failures), which makes their management more challenging. In such consultant cases, achieving by the definite beneficial management strategies that therapeutically address all concurrent comorbidities are really hard to reach or even frequently impossible. ⋯ We can conclude vascular injuries may be the most common cause of severe dermatologic manifestations of COVID-19, which is concordant with many proposed hypercoagulation tendencies and systemic inflammatory response syndrome as one of the most important pathomechanisms of COVID-19 so the skin may show these features in various presentations and degrees.

The enormous spread of the novel Corona virus disease (COVID-19) represents a challenge to dermatological practice. Accumulating evidence has suggested a possible role of teledermatology in facing this challenge. In this article, we aimed to give a general overview of teledermatology in terms of models of practice, modes of delivery, advantages, limitations, ethical considerations and legislative challenges as well as discussing, using examples from literature, how dermatological practice can benefit from teledermatology during the time of the COVID-19 pandemic. ⋯ Teledermatology could be an accessible, accurate and cost-effective substitute for conventional face-to-face dermatological consultations during the COVID-19 pandemic. However, teledermatology practice needs updated legislation and guidelines. More efforts should be done to encourage dermatologists, especially in underserved communities, to provide teledermatology services. Ethical issues and data security related to teledermatology have to be considered.
