A residual intra-articular varus after medial opening wedge high tibial osteotomy (HTO) for varus osteoarthritis of the knee.
Varus deformity of knee osteoarthritis was formed by both intra-articular and extra-articular pathologies. Such intra-articular deformities could not be fully corrected by a medial open-wedge high tibial osteotomy (HTO), which was performed as an extra-articular procedure. Therefore, the purpose of this study was to investigate whether any residual varus was left inside the joint after HTO in the patients with knee osteoarthritis, and a correlation of the residual varus could be traced. ⋯ The capability of HTO to correct intra-articular varus deformities, which was represented by JLCAs, is limited. Postoperative residual JLCAs were correlated primarily to preoperative JLCA values and total alignment correction, while the former accounted for most. A preoperative JLCA of 6° was suggested to be a tipping point, and a larger value indicated more than 5° residual JLCA after the HTO.

Kinematically aligned total knee arthroplasty reproduces native patellofemoral biomechanics during deep knee flexion.
The implant positioning for kinematically aligned total knee arthroplasty (TKA) differs fundamentally from conventional mechanically aligned TKA. This difference may affect patellofemoral (PF) biomechanics after TKA. This cadaveric study tested the hypothesis that kinematically aligned TKA would restore PF biomechanics to the native condition better than mechanically aligned TKA. ⋯ Kinematically aligned TKA better restores patellar kinematics and PF contact pressure distribution to the native condition than mechanically aligned TKA during deep knee flexion. These findings provide clues to understand why kinematically aligned TKA is associated with less anterior knee pain and better PF functional performance compared to mechanically aligned TKA. Patients undergoing kinematically aligned TKA may experience a more normal feeling during deep knee flexion activities.

Severe varus knees result in a high rate of undercorrection of lower limb alignment after opening wedge high tibial osteotomy.
The purpose of this study was to clarify the indication for opening wedge high tibial osteotomy (OWHTO) in terms of lower limb alignment to achieve satisfactory clinical results. ⋯ Based on these results, we recommend that a preoperative FTA of <185° should be included as a criterion for OWHTO alone.

Ilizarov bone transport and treatment of critical-sized tibial bone defects: a narrative review.
Critical-sized bone defects of the tibia are complex injuries associated with significant problems that are difficult to treat, and they are associated with a significant burden of disease in clinical practice; however, the treatment of these cases has still been a challenge for orthopedic surgeons. The aim of this review was to evaluate the current available studies reporting on classical Ilizarov methods in the treatment of infected or noninfected critical-sized bone defects of the tibia, and to perform an analysis of treatment period and complications. ⋯ V.
