Simvastatin for the prevention of exacerbations in moderate-to-severe COPD.
Retrospective studies have shown that statins decrease the rate and severity of exacerbations, the rate of hospitalization, and mortality in chronic obstructive pulmonary disease (COPD). We prospectively studied the efficacy of simvastatin in preventing exacerbations in a large, multicenter, randomized trial. ⋯ Simvastatin at a daily dose of 40 mg did not affect exacerbation rates or the time to a first exacerbation in patients with COPD who were at high risk for exacerbations. Surgical volume impacts bariatric surgery mortality: a case for centers of excellence.
Concerns regarding care quality prompted credentialing processes for bariatric "Centers of Excellence" (COE). It is hypothesized that high-volume surgeons and hospitals have better outcomes. ⋯ In Pennsylvania, high volume is associated with decreased mortality and LOS. The results support the use of surgical volume in the COE credentialing process.

Estimation of the dietary requirement for vitamin D in healthy adults.
Knowledge gaps have contributed to considerable variation among international dietary recommendations for vitamin D. ⋯ The range of vitamin D intakes required to ensure maintenance of wintertime vitamin D status [as defined by incremental cutoffs of serum 25(OH)D] in the vast majority (>97.5%) of 20-40-y-old adults, considering a variety of sun exposure preferences, is between 7.2 and 41.1 microg/d.

Multicenter Study Comparative StudyNurse involvement in end-of-life decision making: the ETHICUS Study.
The purpose was to investigate physicians' perceptions of the role of European intensive care nurses in end-of-life decision making. ⋯ Physicians perceive nurses as involved to a large extent in EOLDs, but not as initiating the discussion. Once a decision is made, there is a sense of agreement. The level of perceived participation is different for different regions.

To assess the efficacy and safety of gabapentin in patients with fibromyalgia. ⋯ Gabapentin (1,200-2,400 mg/day) is safe and efficacious for the treatment of pain and other symptoms associated with fibromyalgia.
